Caleb Gallifant serves as Chief Operating Officer of CenterWell Home Health (CWHH) and is accountable for CWHH’s clinical and business operations, including all skilled home health operations, efforts to increase clinician capacity, and the expansion of post-acute services.

As COO, Caleb is accountable for driving the evolution from fee-for-service to value-based payment models to improve health and business outcomes and strengthen the long-term financial health of CWHH.

Previously, Caleb led Integrated Health, a division of Humana dedicated to delivering affordable health plan products and coordinated, comprehensive, and proactive care to improve outcomes and experiences for members and patients.

Prior to his role with Integrated Health, he co-led the strategic evaluation and divestiture of Humana’s Employer Group business, following his tenure as Vice President of Product Development and Strategic Partnerships, where he oversaw the Commercial Medicaland Specialty product portfolio and key strategic partnerships.

Caleb also managed Commercial Digital and Consumer Health Products, including On Hand, the industry’s first virtual primary care plan enabled by a custom mobile experience. He holds a Master’s of Healthcare Delivery Science from The Tuck School of Business at Dartmouth College, and a bachelor’s degree from Baylor University. He was a Nashville Health Care Council Fellow in the 2022 class.
